This policy explains what information the Zain Family Dentistry website collects when you visit or send us an appointment request, how we use it, and the choices you have.

The short version

• We only collect what you choose to send us through the appointment request form, plus the basic technical information every website receives.
• We use it to reply to you and to keep the website secure. We do not sell it and we do not use it for advertising.
• We do not run advertising or analytics trackers of our own. One outside tool on this site, Cherry’s payment-plan estimator, measures its own use — see “Cookies and similar technology” below.
• Email and web forms are not a secure way to send health details, so please keep those for a phone call or your visit.

This policy and your dental records

This policy covers this website only. The way we use and protect your health information as your dental provider is governed by federal privacy law (HIPAA) and is described in our Notice of Privacy Practices. You can ask for a copy at the front desk or by calling (614) 279-0674.

Information you give us

When you send an appointment request, the form asks for:

• your first and last name
• your email address and phone number
• the type of visit and the day you prefer, if you choose to say
• any message you write

The form is delivered to our office by email so that we can contact you. It is not stored in a database on this website. If you call or email us directly, we receive whatever you choose to tell us.

Please do not include medical, dental or insurance details in the form or in an email. We will take those from you securely by phone or in person.

Information collected automatically

Like every website, ours receives some technical information when your browser loads a page:

• Server and security logs. Our web host and our security provider record your IP address, the browser and device you are using, the pages requested and the time. These logs are used to run the site and to investigate abuse.
• Spam protection on the form. To stop automated spam, the form runs a check that you are a person, and we keep a scrambled, non-readable record of the sending IP address so that we can limit repeated submissions. Blocked attempts may be logged with the IP address they came from.

Cookies and similar technology

We do not set advertising or analytics cookies of our own, and we do not track you across other websites. Three outside services on this site may store information in your browser:

• Cloudflare, which protects the site and the form, may set cookies that are strictly necessary for security.
• Google Maps, which shows our location, may set Google’s cookies when the map loads.
• Cherry, which provides the payment-plan estimator, stores an anonymous identifier in your browser and uses an analytics service (Segment) to measure how its tool is used. It does not load on our Contact page.

You can block or delete cookies and site data in your browser settings. The site will still work, although the form’s security check may ask you to do more.

Other companies involved

We rely on a small number of outside services to run this website. Each receives only what it needs to do its job:

• Cloudflare — website security and delivery, the “are you a person” check on the form, and the icon files the pages use.
• Our web hosting provider — stores and serves the website.
• Zoho Mail — our email provider, which delivers appointment requests to our inbox.
• Google — the fonts used on the site and the embedded map. Your browser contacts Google to load them.
• Pexels — hosts some of the photographs on our Dental Care page. Your browser contacts Pexels to load them.
• Zocdoc — online booking. The “Book Appointment” button takes you to Zocdoc’s website, where Zocdoc’s own privacy policy applies.
• Cherry — payment plans. The payment calculator on our Payment Plans page and the “Pay over time” button shown on most other pages are provided by Cherry and load from Cherry’s servers. Cherry and its analytics provider therefore receive the same basic technical information as any website you visit, along with which page you were on. If you apply, you apply directly with Cherry and Cherry’s own privacy policy applies. We do not see the financial details you give them.

We are not responsible for the privacy practices of other websites we link to. Please read their policies before giving them information.

How we use your information

• to reply to your request and arrange your appointment
• to answer your questions
• to keep the website and the form secure and working
• to meet our legal obligations

We do not sell your personal information, and we do not share it with anyone for their own marketing.

How long we keep it

Appointment requests are kept in our office email for as long as we need them to respond to you and to maintain our records as the law requires. The spam-protection records described above hold no readable personal details. Server and security logs are kept by the providers for their standard retention periods.

Security

This website uses an encrypted (HTTPS) connection. No website or email system can be guaranteed completely secure, which is why we ask you not to send health information through the form.

Children

We care for patients of all ages, but this website is intended for adults. Parents and guardians are welcome to request appointments for their children. We do not knowingly collect information directly from children under 13 through this website.

Your choices

You can ask us what information we received from you through this website, and ask us to correct or delete it. We will do so unless we are required to keep it. To make a request, contact us using the details below.

Changes to this policy

If we change how the website handles information, we will update this page and the date at the top.
